HOW TO APPLY FOR A DISABILITY PARKING PERMIT
Disability parking permits must be applied for in the name of the disabled person or by the parent or guardian of the disabled person. The permit is for the exclusive use of the disabled person and may be used in any vehicle in which the disabled person is riding. There are two types of permits – temporary and permanent. Both require certification from a medical professional. The applicant must submit a properly completed application requesting either a temporary parking permit (DMV form ‒ B-225T) or a permanent parking permit (DMV form ‒ B-225P).
OBTAINING FORMS
Application forms can be obtained at any Department of Motor Vehicles office, online at https://dmv.service.ct.gov/CustomerOnlineServices/s/disability-placard?language=en_US, or by mail by contacting the DMV Phone Center at 860-263-5700 (Hartford area or outside of Connecticut) or at 800-842-8222 (Toll free within Connecticut only).
SUBMITTING FORMS
The forms can be submitted electronically (https://portal.ct.gov/dmv/resources/dmv-forms?language=en_US), returned by email: (dmv.hpapp@ct.gov), or in person at any DMV office or mailed to: Department of Motor Vehicles, Disability Placards, 60 State Street, Wethersfield, CT 06109. Permits for a temporary disability have a $5 application fee and are valid for up to six months. A permit for a permanent disability has no application cost. Permits for a permanent disability are valid for the same term as a driver’s license or identification card (typically six years) and must be renewed. DMV will send to the permit holder a renewal notice approximately 45 days prior to expiration. Motorcycle riders can apply for a license plate with the International Symbol of Access. These plates are issued only to a motorcycle registered in the name of a person with a disability.
TEMPORARY PERMIT
Temporary disability parking permits are provided for people who are temporarily disabled and are valid for a period of six months. An application must be filled out (DMV form – B-225T) for a permit to be issued. There is a $5.00 fee for a temporary permit.
